Define the Health Navigator role and its significance in a rural community.
Detail the three primary objectives of Triple Aim.
Describe the Health Works Model; how does expansion to contiguous communities address rural healthcare gaps?
Directions for Mini-Papers:
On the identified weeks students are expected to write a mini-paper response to the questions asked in each of the mini-paper prompts. This paper will not exceed 1 1/2 page double-spaced 12 pt. Times New Roman font with 1” margins. Topics may include case studies to give students an opportunity to demonstrate their understanding of and ability to explain issues. Students should be able to understand and apply concepts from the readings when analyzing the material. Students must respond to all aspects of the question(s) posed in the mini-paper prompt to receive full points. APA style is an expectation for citations and references.
