Much of clinical care today is practiced in teams, and coordinating the care delivered by teams is a major challenge. Thinking in terms of the EHR functional components, (a) what is at least two ways that EHRs can facilitate care coordination and (b) what are at least two ways in which EHRs are likely to create additional challenges in care coordination.
