Your community health emergency preparedness assessment should answer the following question: What is the ability of your chosen community to meet the nursing care needs of populations affected by priority health issues during disaster situations? You will use the windshield survey (community health profile) to help you complete this project.

Specifically, the following critical elements must be addressed:

I. Introduction: Provide an overview of the community you chose to study, including geographic location and demographic information such as population(s) and socioeconomic information.

II. Assessment: In this section, you will analyze the health indicators of the selected community.

A. Health Profile: Summarize your findings from the windshield survey that you previously completed.

B. Overarching Goals: Discuss your chosen community with respect to the four overarching goals of Healthy People 2020. In other words, how does the community account for a wide diversity of populations with regard to responding to the Healthy People 2020 goals initiative? Be sure to defend your response.

C. Implementation: Analyze the implementation of Healthy People 2020 goals within your chosen community. In other words, based on four of the Healthy People 2020 goals, what evidence exists to indicate your selected community is making progress toward achieving these overarching goals? Be sure to discuss both areas of strength and areas of need. Be sure to support your claims.

D. Social Determinants of Health: Based on your analysis of the implementation of the Healthy People 2020 goals and the health profile of the community, identify the social determinants of health of the specific community, justifying your identifications with technological data analytics.

E. Priority Health Issue: Based on your analysis of these areas of strength and need, identify the priority health issue for your chosen community.

III. Diagnosis

A. Community Nursing Diagnosis: Develop a comprehensive community diagnosis (negative or positive) related to the priority health issue and social determinants you identified. Include a clear statement of each of the following. Justify your response with technological data analytics.

i. Risk situation

ii. Population characteristics related to the risk

iii. Evidence of elevated risk for the population

B. Epidemiology: Based on your community diagnosis, how do you see epidemiology playing a role in the emergency preparedness for the priority health issue? Be sure to support your claims. In your response, you may consider patterns, causes, and effects of the disease or condition.

IV. Outcomes and Planning: Review your chosen community’s emergency evacuation plan or program, specifically with regard to the community nursing

diagnosis you developed and the nursing concerns and needs of the affected population. Complete the following based on your review:

A. Type of Disaster: Based on your review of the community’s emergency preparedness plan or program, as well as community and national databases, what type of disaster is your community most likely to experience?

B. Mandatory Evacuation: Analyze the nursing care needs of the population affected by the previously identified priority health issue in terms of a mandatory evacuation following the disaster you identified. Ensure you consider both environmental and epidemiological needs. For example,

what would patients need, at a minimum, to ensure their nursing care needs are met? What supplies, equipment, and/or services might be necessary to ensure patient safety and survival? What must be considered if these patients have pets or service animals?

C. Agencies of Care: Based on your assessment, identify those community agencies of care (fire department, police, medical professionals, public health, transportation, etc.) that would be available to assist this population during an evacuation.

D. Meet Needs: Based on your previous identifications, to what extent do you feel the community is able to meet this population’s nursing care needs during a mandatory evacuation? Be sure to justify your response with evidence and examples.

E. Collaborate: Explain how you will apply nursing theories to effectively collaborate with the previously identified community agencies in meeting the needs of the community. Be sure to support your claims.

F. Agencies Address Determinants: How do the identified agencies of care in the community help address the previously identified social determinants of health? Be sure to support your claims.

V. Implementation: Review your chosen community’s emergency evacuation plan or program, specifically with regard to the community nursing diagnosis you developed and the nursing concerns and needs of the affected population.

A. Ride Out the Storm: Analyze the nursing care needs of the population affected by the priority health issue who might choose to ignore the mandatory evacuation and “ride out the storm.”

B. Might Meet Needs: Based on your assessment, identify the agencies of care in the community that might be able to assist the population that chooses not to evacuate, and assess the extent to which you feel the community might be able to meet the nursing care needs of this particular population. Be sure to substantiate your claims.

VI. Evaluation

A. United Nations Sustainable Goals: Reflect on how the previously identified community diagnosis of health applies to the United Nations (UN) sustainable goals. Specifically consider how the nursing care needs of the community relate to the UN sustainable goals. Be sure to support your claims.

B. Global Scale: Choose one element of Maslow’s hierarchy of needs to consider, and reflect on how your emergency preparedness plan may or may not work on a global scale. Specifically consider both social and political aspects.

C. Culture: Discuss how culture plays a role in implementing this emergency preparedness plan, applying the cultural competence theory. What are some cultural barriers to care that may influence the community being assessed? Consider interventions that may assist with overcoming these barriers.

VII. Conclusion: Summarize your assessment of your selected community and its ability to provide for its affected populations in a disaster situation.
